Crossriver Quality Health Partners is a provider-led and governed Clinically Integrated Network that relies on a collective effort to improve healthcare outcomes across our population. By collaborating, we will lead the transformation from volume-based reimbursement to value-based compensation. You, as an active participant in Crossriver, will help to: shape the organization; improve the quality of care; improve patient satisfaction and provider satisfaction; and, improve the cost-effectiveness of healthcare in to our communities.
Together, the provider-members will:
Implement evidence-based clinical guidelines for prevalent diagnoses/disease states with the goal of lowering cost and improving quality
Communicate clinical guidelines to the entire network
Develop a comprehensive patient wellness program
Manage patient health across the clinically integrated network
Negotiate payments recognizing providers’ efforts and patient outcomes
Reduce the total cost of care for patients, employers, and payers
